Assertive Community Treatment (ACT) Teams

MHR started four Assertive Community Treatment (ACT) teams in 2005: two in Ramsey County (now consolidated into one larger team), one in Hennepin County and one in Dakota County. Each Team functions as the primary treatment provider in which services with clients are proactive, targeted, comprehensive and individualized. All four teams were developed from existing intensive teams.

ACT teams utilize a consistent teaming approach and meet specific "fidelity standards" with regard to composition. They have a low staff to client ratio (1:10) which allows team members to see clients frequently (several times per week) in order to attend to the higher needs presented.

ACT teams assisted 274 MHR clients with psychiatric evaluations, medication education and monitoring, education about symptom management and recovery, family psychoeducation, vocational planning, connections with other healthcare providers and rapid response to client crises.
