Targeted Case Management Teams (TCM)

There are four Ramsey Targeted Case Management (TCM)
teams, which include 32 staff members. In Dakota County there is one TCM team which includes 8 members.
These teams help people
with serious and persistent mental illnesses gain access to needed medical,
housing, social, educational, financial, vocational and other
services necessary to meet their mental health needs.
The team members typically have 30 or more clients on their
caseloads, who they see monthly or more frequently if needed.
These teams served 1068 clients in Ramsey County during 2005.
In Dakota County, the Case Management Team served almost 200 clients at ratio of 1:18, with one position
designed to serve 35.
