MHR values the experience of each person and partners with them to develop a plan to address their unique needs. While participating in MHR services, people experience support for their physical and mental health issues to assist them on their path to recovery. We believe people with mental illness and substance use disorder should have equitable access to resources, support and opportunities to achieve their highest level of recovery.

Supportive Services

Mental Health Outreach Clinic (MHOC)

The Mental Health Outreach Clinic (MHOC) provides in-home therapy to adults who live with serious mental illness. Therapists in the Mental Health Outreach Clinic are licensed as psychologists, clinical social workers, and marriage and family therapists. MHOC is designated as an Essential Community Provider (a health care provider that serves high-risk, special needs, and underserved individuals).

Our therapists have experience working successfully with individuals who live with mental illness and also see couples and family members who are impacted by mental health and or substance abuse issues. MHOC therapists provided services to people living in the Twin Cities metro area. Appointments are available days, evenings and weekends.

Health & Wellness Initiative

In 2006 NASMHPD released a landmark publication that showed that people with serious mental illness die on average 25 years earlier than the general population. This is largely due to treatable medical conditions like lung disease, heart disease, and diabetes. The people we serve experience health inequities related to multiple social determinants of health, such as stigma, lack of health care access, lack of transportation, poverty, racism, homelessness, and unemployment. They have limited access to necessary supports, such as treatment for tobacco use and dental care. MHR feels strongly that it is our responsibility to not only help individuals address needs related to their mental health and substance use, but to also help them with their overall health. We cannot accept these health disparities and are committed to changing this.

Since 2014, MHR has committed resources to address these alarming disparities through our Health and Wellness Initiative. We have worked to build health and wellness directly into our services, policies, and environments. Our Health and Wellness department provides training and resources to MHR staff so that they can better assist in meeting the health needs of the people we serve. Some of our current initiatives include supporting individuals to address commercial tobacco use and accessing dental care. Through this work, we are changing a culture of neglect of integrated care that is pervasive in behavioral health and helping the people we serve improve their overall health. And we are seeing the impact of our commitment. In 2021, the Constellation Fund calculated a $11.67: $1 benefit/cost ratio for the services we provide regarding Health and Wellness for the people we serve.
